During our business hours

During 1st visit, you will be asked a variety of questions regarding your health by Dr. Moshtagh: face to face or by Telemedicine 📞: no difference between both of them !

This will take an average of 35-60 minutes depending on the complexity of your condition.

After your visit, the doctor will go over her thought processes as to the CAUSE of your condition and will get you started on your plan by:

1. Ordering supplements to your doorstep.

2. If you require bloodwork, your requisition will be emailed or mailed to you. If you require test kits, these will be mailed to you as well. You will mail to the respective lab facility.

COVERAGES:

• PPO insurances with low or no deductible may cover your visit(s) and blood work costs. We will provide you with a Receipt called a (Super-bill) that you can submit to insurance for reimbursement.

• Please provide us with the following information:

If you have PPO, what is your deductible for blood tests for out of and in network doctors. Please call your insurance and have this information ready at time of the visit ***

COSTS:

• Initial visits are $299 depending on different services offered

• Cost covers: Doctors time and expertise to:

o Review of your symptoms and history , history collection, lab analysis if any done in part 3 months and putting together a plan including blood work order according to your insurance, possible referrals, and natural therapies if acute case
